Voluntary Self-Exclusion Program
Run by: Iowa Racing and Gaming Commission
Covers: Licensed casinos and racetracks, sports wagering and fantasy sports operators (and operators approved in future). Terms: five years or for life; a 5-year term ends automatically and cannot be ended early; a lifetime term signed on or after 07/01/2017 cannot be ended; those who enrolled on or before 06/30/2017 can request revocation.
How to enroll: Complete and submit an enrollment form: by mail (notarized, with ID copy and photo), in person by appointment at the IRGC Des Moines office (515-281-7352), at an Iowa Gambling Treatment Program agency, or at any state-licensed casino or racetrack. The term begins on the date next to the signature; licensees can take up to 7 calendar days (the program page says up to two weeks) to enforce it. No cost.
